Porcelain is This Season's Top Product but Research Never Stops for the Next Big Thing

By: Joseph Hopkins

Ironically, because of the success of their attempt to reduce tooth decay by almost 40 percent, the American dental industry is now faced with a big problem at the start of 1980s Just as the demands for general dentistry went down, cosmetic dentistry has begun to blossom. Today, more than 80 percent of the nation's 130,000 dentists do cosmetic work, and many report it is the fastest growing part of their practice.

The cosmetic division of dentistry is said to be able to generate almost half of the $32 billion annual revenue of the industry, said a professor at a medical college in Georgia and book author. When placed side by side to what cosmetic work made five years ago, cosmetic work has increased three times, according to the president of the American Academy of Cosmetic Dentistry. Because of the new and more improved materials that they have, the great increase was made possible.

Porcelain laminates are probably the most apparent among these enhanced materials, and they are thin porcelain veneers that are applied on existing teeth and bonded in place. So that chipping will be prevented, patients should not bite hard, like on spareribs and even their own nails. Laminates are low maintenance yet high in durability and it looks very real. The revenue of dentists who have no less than 10 treatments with porcelain laminates a month reaches $250,000 a year.

Bleaching will join the list of the popular cosmetic dental treatments in the US today and it is a chemical whitening procedure that costs around $100 to $200 per treatment. Most of the clients who avail of this work are the entertainers and politicians. The private individuals patronize this also, particularly the men. From 20 percent a few years ago, men now comprise 40 percent of patients who consult for cosmetic procedures.

Although dentists have long been using porcelain, it was only recently that porcelain veneers, and the covering technique to current teeth has been fully refined. The sales for porcelain laminates for this year alone will reach 300,000 amounting to $20 million for the American manufacturers. The top of the line material these days is porcelain, but dental cosmeticians have yet to cease hunting or baking to find a better bond. One dentist uses ceramic particles in a resin base to form caps and bridges. The caps are baked in an oven for seven minutes after being formed in the mouth. You will come up with stronger caps and for a price that is half of the porcelain laminates.

If there is a specialty designed oven, the one that costs $1000, dentists can definitely perform baked bonding on site. As one of the dentists is baking, another New York dentist has designed a patented silk bonding technique that he claims to lift up bonding strength three times. Unlike the regular bonding, however, it is 25 percent more expensive. It has been acknowledged by an official of the ADA that the baking option used can indeed strengthen the composite materials, but not that significant.

Cutting out the need for bonding in certain cases is another new process being used. At present, they are just putting hydrogen peroxide to whiten them. Discoloration that has been bleached away may recur, but dentists say the procedure has yet to have an ill effect reported. One problem cosmetic dentists have is that patients have trouble visualizing the result of cosmetic alterations and often back off because of uncertainty.

In Southern California, a university professor has formed an electronic system where he can show his patients the expected outcome of the dental cosmetic work. The system is an interplay of a video camera, an electronic pen and an electronic writing pad, hooked up to a tower with a color motion picture display. Before being fed to the monitor, the image of the patient's smile is captured using the camera. Using the computerized pen and pad, he can display the cosmetic enhancements onscreen by whitening the teeth, filling in gaps or refining rough edges.

They way up close smile almost always surprise the patients. Seeing the great improvement in appearance for only the slightest change will amaze them even more.
